Do you give consent for your child leave the session unaccompanied?
If your child is in Yr 3-6, we anticipate a parent or guardian will drop them off and collect them at each session. However, if you are happy for your child to attend unaccompanied please indicate above. If your child is in Yr 7-11 we are happy for them to attend unaccompanied. However, if you would like to drop off and collect your child please indicate above.
Please provide the name of individuals authorised to collect your child.


Consents

Occasionally, we will ask a photographer to visit a group to take pictures of the session which we will then use on social media and other publicity. These will never have a child’s name attached to them. If your child is under a child protection order, please indicate No below.

If we are approached by another media outlet, for example local radio, we will seek further and separate consent from you.
Do you give us permission to take photos and videos for use in marketing material, and on our website and Facebook?
Are you happy for your child to take part in visits within the venue? e.g to look at an exhibition in the venue or visit the archive.
May we use your child’s image in material that we produce for promotional purposes, such as display boards or banners?
May we record your child’s image on video or voice on an audio recording? We may make a video or podcast featuring your child as a way to remember a fun activity.
We expect everyone associated with our activities to behave responsibly. We will alert you if we have any cause for concern and we reserve the right to temporarily or permanently exclude anyone whose behaviour is unacceptable. We do not tolerate any form of offensive behaviour.
